Scheduling an Appointment
Appointments can be scheduled over the phone or in person. Please note that It is the responsibility of the patient to remember their appointment information. No reminder calls will be made prior to appointments. Email confirmation will be sent if you have provided us with a valid email address (coming soon).
Please note, you will be asked why you are requesting an appointment. This is to help our staff find you the most appropriate appointment. Please be honest with our staff on the phone so that we can make sure you are receiving the appropriate care.
Missed Appointments/Cancelling Appointments
Missed appointments will result in a Missed Appointment Fee corresponding to the type of appointment missed (ie longer appointments like Physicals will result in a higher fee).
We require 24 hours notice for cancellations. Any cancellation with less than 24 hours notice may be subject to a Missed Appointment Fee.
Should you arrive late for your appointment, your doctor may no longer be able to see you and you may be asked to reschedule. Additionally, a Missed Appointment Fee may apply.
Urgent Appointments
Often a minor problem such as a bladder infection, ear ache, sore throat or minor injury such as a sprained ankle may occur overnight. We ensure that there are urgent spots available daily for such problems. We have a limited number of spaces each day, to ensure we can meet your needs, please call early in the day. Additionally, if your doctor is not available then you may be offered an urgent appointment with one of our other doctors in our urgent clinics or with the on call doctor.
Please note, these appointments are NOT meant for follow ups, lists of concerns, chronic issues, check ups, or other non-urgent concerns.
Refilling Your Prescriptions
If you take regular medications, please make sure you call well in advance of running out of medication to book an appointment with your doctor for refills. Renewal of routine medication will not be viewed as a reason for an urgent appointment. We do NOT refill medication over the phone.
Please note, Narcotics and controlled substances will NOT be renewed by phone or fax.
If your pharmacy has faxed a renewal request, please all two (2) business days for a response.
Form Completion
The College of Physicians & Surgeons of Ontario expects physicians will complete and submit third party reports within sixty (60) days. We ask that forms be dropped off in advance of booking an appointment with your doctor for review. Also, the patient must complete their portion of the form in advance, otherwise, it will not be completed by your physician. When completed, you will be contacted accordingly. If you have not been informed after thirty (30) days, please call us to inquire on the status of your form.
We accept cheques, debit cards, Visa or Mastercard.
Our Services and Impact of Outside Usage
As a member of our Family Health Organization (FHO) you are encouraged to use our facility for your medical needs. As you look for medical care outside our FHO, it should be noted that OHIP charges your doctor a penalty fee. As a patient of our FHO, you must see your family doctor unless he/she is away from the office for an extended period or if it is a medical emergency and your doctor is unavailable. If you have an urgent medical problem outside our regular hours, we ask that you call the Tele-Health Assistance System (THAS) line at 1-866-553-7205, or, if a medical emergency, go to the nearest Emergency Department at your nearest hospital.
